This coverage is required for participants under age 50. Participants over age 50 are
strongly encouraged to participate, but may instead provide proof of individual coverage.
To enroll, you may either:
a) Complete the above enrollment form and mail it to the address listed on the form.
b) Visit HTH Worldwide and enroll online.
Enter Miami's group access code CNX-4745 under "Have a group access code?" to begin.
The rate for participants under age 50 is $10.50 per week. For those over 50, the rate is $42 per week.
Please use May 17 and May 30 as your dates of coverage. If you plan to travel independently before or after the tour,
you may purchase coverage for those additional dates as long as the majority of your travel is part of the study tour.
After successfully enrolling, you should receive both an email and a hard copy confirmation of coverage.
TRAVEL DOCUMENTS
You are required to have a valid passport. Note that some countries require your passport to be good for at least 3 months after your intended stay.
