Liability Release Form
These forms should be turned in to 108 RSC prior to activity.

Vehicle Owner Acceptance of Responsibility
Any time club participants will be traveling using personal vehicles, owners of the vehicles need to fill out this form and turn into 108 RSC.

Travel Notification
This electronic notification needs to be completed at least 3 days before any away event. Click on the link, complete the info and send.

Supervisor Coverage Request
This electronic notification needs to be completed at least 7 days before any home event. Click on the link, complete the info and send.

General Event Template
This form needs to be completed one month prior to any large event.

Accident / Injury Report Form
This form needs to be completed within 24 hours of any injury and turned in to 108 RSC.

Coaching Contract (Non-Student)
This form and a meeting between your coach and the Director of Club Sports should be completed prior to your first practice.

Coaching Contact (Miami Undergraduate or Graduate Student)
This form and a meeting between your coach and the Director of Club Sports should be completed prior to your first practice.

Semester Report
Complete and submit to 108 RSC at the end of the semester.

Annual Report
Completed and submit to 108 RSC upon completion of the season / year.
