Funding Manual
The club sport funding deadline is Wednesday, September 19 at 5p.
Funding application forms will be emailed to club officers in late August.
Club Sport Event Calendar
This calendar will include all the club events.
Compliance Database
Check this periodically to check on the status of mandatory forms, meetings,
and deadlines. The first update will come in early September.
Liability Release
Form
These forms are turned in to 111 Withrow Court.
Emergency
Medical Form
Emergency Medical forms are to be kept by the club president or coach
at all times.
Personal Vehicle
Usage Form
Any time your members will be traveling using personal vehicles need to
fill out this form and turn into 111 Withrow.
Travel
Notification Form
This form needs to be completed at least 3 days before any away event.
The easiest way to complete this form is save it on your computer with
the necessary information then email it to arnosmp@muohio.edu.
Advisor Agreement
This form and a meeting with your advisor should be completed prior to
your first practice.
Coaching Contract (Non-Student)
This form and a meeting between your coach and Mike Arnos should be completed
prior to your first practice.
Coaching
Contact (Miami Undergraduate or Graduate Student)
This form and a meeting between your coach and Mike Arnos should be completed
prior to your first practice.
Event
Planning Form
This form should be completed prior to any large event or tournament.
A meeting to discuss the event should be scheduled with Mike Arnos one
month prior though facilities can be booked up to one year in advance.
