Course and Curriculum Approval Process

- Below are brief descriptions of each form and a link to them. Click on the link to begin the process.

New Course - Use this form when you are proposing a new elective or required course that will be offered permanently within your department.
Major Change to an existing course - Use this form for these substantial changes to an existing course:
- Increase in credit hours
- Modifying course content
- Modifying course number outside of current level (example: from 300 to 400)
- Modifying the grade mode (example: from Credit/No Credit to standard letter grade)
- Increasing variable credit hours outside the maximum (example: current 1-3 credit hour course; 9 maximum changing to 1-5 credit hours; 15 maximum)
- Add or Delete concurrent course(s)
- Modify course instructional type (example: change from lecture/lab to lecture only)
NOTE: If making a Major change AND Minor changes to the same course, you may submit all of them on the Major change form. If topic isn’t addressed in check box, then add to “other” section on Major change form.
Minor Change to an existing course - Use this form for these types of changes to an existing course:
- Title change
- Reduction in credit hours (new syllabus must be attached)
- Modifying course number within the current level (example: HST 312 to HST 332)
- Add or Delete a prerequisite
- Modify semantics of catalog description (if changing the course description modifies the course content, you should instead complete a MAJOR change form)
Temporary Course - Use this form when submitting a course offering on a “trial” basis. This should be used when offering a course that may or may not become a new course after it is taught.
- These courses, when approved, are offered for three consecutive semesters only.
- If course needs to be offered beyond the three semesters, then a new course request form must be completed.
- If the topic of a course is relevant to an existing Special Topics course within the department, then a temporary course request should NOT be used. See section on this website about Special Topics courses.
New Degree Program - Use this form when submitting a proposal for a new degree, major, minor, concentration, emphasis or focus.
Revised Degree Program - Use this form when submitting a request to revise any REQUIREMENT within a major, minor, concentration, emphasis or focus.
- If you are adding or deleting a course within a list of elective courses, no form is required. Just send an email to courseapproval@muohio.edu with the additions or deletions and someone will contact you with questions or to notify you of the update.
Special Topics Courses - The purpose of Special Topics courses is to have an approved course with a broad topic which allows for multiple courses to be offered during a term (example: Topics in American Literature.) Click on the bolded title to learn more about existing special topics courses.
- To propose a new special topics course, complete the new course request form.
Bulletin Updates - When forms from this website are completed and officially approved, the information contained within them will be updated in the next publication of the appropriate student bulletin (General or Graduate) by the Assistant Director of Curriculum.
- If you find an error that is NOT related to course or curriculum information in the current bulletins, please email Amy Ansorg at ansorgad@muohio.edu with the following information: General undergraduate or graduate bulletin, the error, the page number and the correction.

Instructions on downloading and completing these forms
The forms are formatted in Microsoft Word so you can download them and complete them on your computer.
- Click on the form in the section above.
- The software will prompt you to download and save the form OR to open it directly into Word.
- You should DOWNLOAD and SAVE the form to your computer!
- By doing this, you will be prompted to “Save As”.
- Rename the document as the DEPT, the course number, type of request and the date (example: HST 312 New Course 091908)
- Save this to your desktop or appropriate drive.
- Open the document from where you saved it and fill it out completely for each request you need to submit.
- Save completed form(s) and attach to an email to courseapproval@muohio.edu.
