Q. Do I have vision insurance?
A. No, not under Miami's plan, but if you visit a doctor in the network for your one routine eye exam per year, you pay the $25 co-pay. Also, several optical centers offer up to 30% discounts on glasses and contacts. Go to Anthem.com for participating centers.

Q. Whom do I call if I want to know if something is covered under the health plan?
A. Call Anthem at 800-887-6055.

Q. How long are my children covered as dependents?
A. Through the end of the calendar year in which they turn age 19 or until the end of the year in which they turn age 24 if a full-time student.

Q. Do I need to submit a new fee Waiver Form for 2008-2009 if I'm receiving it now?
A. Yes, you need to submit a new Fee Waiver Form for each academic year. Submit by June 30.

Q. How do I update my life insurance beneficiary?
A. Click on the "Forms" link on this website to download and print the form or contact Benefit Services for a Beneficiary Change Form.

Q. How do I find out if a doctor is in the Anthem network?
A. Call Anthem at 800-887-6055 or search via their website at www.anthem.com. Miami's plan is Blue Access PPO.

Q. I just had a new baby. How do I add him/her to the insurance?
A. Click on the "Forms" link on this website to download and print the form or contact Benefit Services for a Health Change Form. This must be done within 31 days of the birth.

Q. My spouse lost his/her health coverage? How can I add him/her to my plan?
A. Stop by Benefit Services to complete the paperwork within 31 days of the loss of coverage. You will need to provide a Letter of Creditable Coverage or Termination of Coverage letter in order to add him/her.

Q. I got hit hard this year on taxes. How can I reduce them for next year?
A. Consider enrolling in a tax-deferred annuity or a flexible spending account (during open enrollment). More information on both these programs is found on this site.

Q. I'm leaving Miami. What do I need to do?
A. Give notice to your supervisor/manager, then come to Human Resources in 15 Roudebush for an exit interview. No appointment needed. We will explain all your benefits, etc.
