Policy for Managing Changes to Application Source Code (Approved)

Changes to application source code in the production environment must be authorized by an appropriate manager of the application using the form below. The appropriate manager in central computing must also approve that the change is a technically sound implementation of the written functional specification or equivalent on the form below. This form may be communicated electronically.

Source code being changed must have proper version control.

Standards stated in "The Application Life Cycle" should be followed prior to sign off by central computing management.

Change Request Form

Through this communication, I request that the following modification be placed in Production:

• Modification identifier
• UniqueID of central computing manager
  Approval date
• UniqueID of person authorizing
• Authorization date
