Checkbox is a Web-only survey tool that supports authoring online surveys on both Windows and Macintosh computers. Faculty and staff deploying a survey for online delivery can quickly learn how to build and deploy a survey using this user-friendly tool.

Request an Account

Faculty and staff can use the Checkbox Account Request form to submit a request for a Checkbox account or for a 'Report Viewing Only' account.

Reminder: To ensure that faculty and staff are familiar with the University data confidentiality requirements, completion of the online 'Survey Tools & Data Protection' training is required before an account can be created for you, or a student you are sponsoring. Training is not required if you are requesting a 'Report Viewing Only' account for Checkbox.

Accessing your Checkbox Account

To access your Checkbox account at the login screen, enter the following URL in the address line of your browser and login using your Miami UniqueID and MUnet password:

Sponsored Accounts

Faculty and staff can request a Checkbox account for students, a student employee or faculty may elect to request student accounts for a class project. These accounts are referred to as 'sponsored' accounts and remain active on a per semester basis.

• Undergraduate student accounts are created with a reduced feature set and will require collaboration with the faculty or staff sponsor.
  
  
• Graduate student accounts are granted full access and require less interaction with the faculty or staff sponsor.
