Visa Assistance

A. Briggs Visa and Passport Expeditor, a service provider with its head office/operation center in Washington DC but with a local Cincinnati office, can assist our students/faculty getting visas for study abroad. They can assist both US students and international students as long as those international students are in the US legally on an extended visa such as a student visa.

The Miami University Global Assistance Program (MU-GAP) supports travelers in a well-coordinated, collaborative effort, with experts from within the university, as well as contracted providers, and specialists in the field.

At this location you can apply for:

• New U.S. Passport Book or Card
• Renewal of your U.S. Passport
• U.S. Passport Book or Card for a minor

 

 

As a service to the community, we will accept your completed passport application and forward it to the Passport Services section of the U.S. Department of State for an issuance determination. Only Passport Services can make that determination, the Passport Acceptance Office cannot.

1. Passport Application Form DS-11 (unsigned)

• Application forms are availalbe at our office 127 McGuffey Hall.

• Printable applications are also available online at www.travel.state.gov

2. Proof of U.S. citizenship (one of the following)

• Previous valid undamaged U.S. passport

• If you were born in the U.S., a Certified Copy of your Birth Certificate issued by the state, city, or county of your birth. A Certified Copy will have a raised, embossed, impressed, or multicolored seal and the date the certificate was filed. Hospital Birth Certificates are not acceptable.

• NOTE: we will send your original documentation with your application. Your orginal documentation wil be mailed back to you by the State Department.

3. Proof of Identity

• Valid Ohio driver's license – if your license is not from Ohio a second photo identification card is also required

For children under age 16, the child and both parents or legal guardian(s) must appear in person and present the following:

• Evidence of the child’s U.S. citizenship
• Evidence of the child’s relationship to parents/guardian(s) AND Parental/guardian identification

If only one parent appears, you must also submit one of the following:

• Second parent’s notarized written statement consenting to issuance for the child

• Primary evidence of sole authority to apply

• A written statement (made under penalty of perjury) explaining the second parent’s unavailability.

4. One passport photo (available at Gaskill Hall, see tab above)

Photos must be:

      • In color

      • 2 x 2 inches in size

      • Taken within the past 6 months, showing current appearance

      • Full face, front view with a plain white or off-white background

      • Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head

      • Taken in normal street attire:

      - Uniforms should not be worn in photographs except religious attire that is worn daily

      - Do not wear a hat or headgear that obscures the hair or hairline

      - If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture

      - Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons (a medical certificate may be required)

Additional Optional Fees

Expedited Service is an additional $60 per application that is to be included in check/money order payable to U. S. Department of State
One Way overnight fee of $12.72 can be included with the check/money order payable to U. S. Department of State
An Additional one way overnight fee of $17.50 can be included in the payment to Miami University to have their application Express mailed to the Passport Facility

How long does it take to get a passport?

Routine Service
As of Friday August 21, 2009, routine applications are processed in approximately 4-6 weeks from the time of application.

Expedited Service
As of Friday August 21, 2009, requests for expedited service, that include overnight delivery to and from the Passport Agency, are being processed in about 2-3 weeks door-to-door.

What should I do if I need a passport immediately?

You should make an appointment to be seen at a Regional Passport Agency only if:

    • The U.S. passport is needed in less than 2 weeks for international travel

    • The U.S. passport is needed within 4 weeks to obtain a foreign visa

Contact the National Passport Information Center to make an appointment or locate a Passport Agency.

 

I already applied for a passport, how can I check the status?

Check on the status of your passport application:

• Online at: http://www.travel.state.gov/passport/status/status_2567.html

• By emailing the National Passport Information Center by accessing the following webpage: http://travel.state.gov/passport/about/npic/npic_896.html.

• Or by calling the National Passport Information Center’s toll-free number, 1-877-487-2778 (TDD/TTY 1-888-874-7793)

• Information required to check on status:

- Last name as shown on the application

- Date of birth

- Last four digits of Social Security Number

Currently, it is taking 5-7 days for applications to be tracked online. It may take less time if you paid for Expedite Service and sent your application via overnight delivery.

 

What is a passport card?

The passport card is a wallet-size card that can only be used for land and sea travel between the United States and Canada, Mexico, the Caribbean, and Bermuda.

The card provides a less expensive, smaller and more convenient alternative to the passport book for those who travel frequently to these destinations by land or by sea.

 

Why is there an execution fee for the passport book and passport card?

First time applicants, minors, and those seeking to replace a lost or stolen passport must appear in person before a person authorized by the Secretary of State to give oaths to verify their passport applications.

In order to offer American citizens convenient locations to apply for a passport, the Department of State authorizes Passport Acceptance Agents to accept passport applications on it's behalf.

The execution fee is to reimburse the acceptance facility for the cost of the service.

Applicants applying for both the passport book and card simultaneously pay only one execution fee.
