Application Procedures and Instructions

A. Student

1. Please complete the front of the application.

2. Please note that the ethnic group is requested for statistical purposes only. Aggregate numbers are reported to meet state and federal affirmative action requirements. Miami University provides equal opportunity in education and employment.

3. Please have your parent or guardian sign the approval statement.

4. Please take the application to your counselor, teacher, or principal to complete the back of the application and for his or her signature.

B. Parent or Guardian

1. Please indicate that your daughter or son has your permission to apply to the Junior Scholars Program by signing the application.

2. No payment is due at this time. Accepted students will be asked to complete an Intent to Enroll Card and Class Registration Form. Fees will be billed after course enrollment.

C. High School Counselor, Teacher, or Principal

1. Please sign and date the Junior Scholars Program Application if the student meets the stated requirements. Also, type or print a brief recommendation or comment in the space provided on the reverse side of the application form.

2. Please enclose an official transcript covering grades earned through the first semester of the current year. Also include any college aptitude test scores, i.e., PLAN, PSAT, SAT, or ACT. We cannot act on a student's application until we have these grades and college aptitude test scores. If English is not your native language then please submit your TOEFL scores or a letter from your English teacher certifying your English language skills. Please note that for non native English speakers, either a college aptitude test score or certification of English language skills is required--not both.

3. Mail the completed application and high school transcript showing test scores to:
