This form is to be completed by all international applicants who are in the U.S., after they are admitted to Miami University. If you are currently in F or J status, this form will be used for the issuance of your I-20 or DS-2019 and to transfer to Miami University through the Student Exchange Visitor Information System (SEVIS). If you are in any other visa type, you must also complete the form.
You must complete Section A, and if you are attending school in the U.S, your adviser or counselor must complete Section B. See directions on the form.
