Procedures to Request Visa Document DS-2019
The department wishing to bring a Visiting International Researcher/Scholar to Miami in J-1 visa status must complete:
and obtain the following materials and information:
- Evidence of the applicant's expertise in her/her field (must have at least a Bachelor's degree or equivalent). If the position is tenure-track, then the applicant is not eligible for J-1, but must apply for the H-1 (for H-1 information, contact Celia Ellison at 529-7268).
- If the applicant will be supported from external (non-Miami) sources, a financial statement from the applicant's sponsor which indicates the amount (in U.S. dollars) that the applicant is to receive and the length of time that these funds will be provided.
- Information regarding the applicant's date of birth, city and country of birth, etc. that is on the Request for DS-2019 form. Do NOT send this form to the applicant to fill out. You must ask them for the information and you must complete the form (please print neatly). The form must have all the correct information before a DS-2019 can be issued.
- If dependents are to accompany the scholar, information is to be entered on the DS-2019 request for dependents including their name(s), relationship to the applicant, birth date(s), etc.
The person who is inviting the scholar is to complete the Request for Temporary Appointment of International Scholar or Researcher as Exchange Visitor(pdf) The department chair approves the appointment by signing the request form and sends it with the Nomination and all supporting documentation to the division dean for signature. These signed forms are then to be forwarded to the Provost's Office for approval. Upon approval by the Provost's Office, a letter of appointment will be prepared by Academic Personnel. Academic Personnel will forward the request form and the original and copy of the appointment letter to the International Student and Scholar Adviser in the Office of International Education (OIE).
Once the appointment has been approved, the International Student and Scholar Adviser will issue the DS-2019 and mail all items directly to the scholar by regular airmail unless the department requests that they be sent via Federal Express (see section on DS-2019 request form for department account number). The Office of International Education will offer advice and counsel to the Visiting Researcher/Scholar on all matters related to international travel documents and the regulations of the U.S. Citizenship and Immigration Service (CIS) and the U.S. Dept. of State, that apply to international visiting scholars. The scholar is expected to report to the Office of International Education upon arrival so that photocopies of his or her visa documentation can be made for the file. SEVIS fee: All applicants for J-1 who are outside the U.S., must pay the $100 SEVIS fee in addition to the visa application fee. For information, go to www.fjmfee.com or to www.muohio.edu/international/faculty-intl.php. J-2 dependents do not have to pay the SEVIS fee.
NOTE: Under U.S. Department of State regulations which govern the administration of the Exchange Visitor Program, all participants and dependents are required to purchase insurance coverage that will provide a minimum benefit of $50,000 per accident or illness, repatriation of remains in the amount of $7500, and emergency medical evacuation in the amount of $10,000. If the researcher/scholar will not be covered under Miami University's group insurance plan, information on coverage offered by other insurance companies can be obtained in the Office of International Education. Those who are covered under Miami's health insurance policy are required to purchase Repatriation and Emergency Medical Evacuation coverage separately.
