In the section "Room/Building,"
provide the location of waste chemicals. Pickups will be delayed if the
submitted forms are incomplete, including if they are not signed.

If you have too many entries to fit on the two pages, please print
more copies of the second page of the form.

EHSO will collect your chemicals
approximately 10-14 working days following the submission of the
request(s). Large pickups or lab cleanouts may require additional
time.
