Submitting New Applications for Review

Prior to attempting to complete an application, you will need to complete training requirements. Refer to the MU guidelines and other references listed below and provided with the application packet for details on completing the application.

The application procedure at Miami is being revised. The applications below have been in the beta test phase since January 2009. This format or the previously used format will be accepted through the fall semester of 2009.

Three types of research have been identified as common at Miami, choose the appropriate application packet to download. Note: selection of the "wrong" packet does not mean your application will be rejected. The issues

addressed in each are essentially the same, the major difference

You will need to know the date you completed the online training, find these dates here:
CITI Completion Dates

between the packets is the guidance that is supplied with each.

Applications for New Research Projects/Protocols:

Type I General research Used for most types of research
Type II Students are Subjects

For use when your students are the subjects in your research, K-12 or post-secondary settings

Type III Students are Researchers Most applicable in classes when student led research projects are part of the curriculum.*
* The "Students are Researchers" application is primarily for class assignments, not for thesis, dissertation, or independent study types of research

Difficulty downloading and opening zip files? Click Here

Each packet (zip file) contains several files: there is a coverpage electronic template, a research description template, and a guidance document (this provides information on how to address the questions posed on the description template). The cover pages and research description are submitted to the IRB. Each are in a generic word-processing format and must be completed using a computer. Hand-written applications will no longer be accepted.

Students cannot submit applications directly to the IRB. Faculty advisors must submit the applications. Consult the links at the upper right of this page for information.

Submit your application to the IRB or to your advisor as two files using your MU email account:

      • File 1: The completed cover-page;
      • File 2: The research description with all attachments (e.g. recruitment flyers, consent form, surveys, etc). File 2 can be sent as either an MS-Word or PDF type document
      • Faculty: attach the files to an email message and send to humansubjects@muohio.edu
